United Way of Bladen County will accept disaster assistance applications from Bladen County flood victims through Wednesday, Jan. 25 at 5 p.m., the organization announced recently. Applications are available at 401 Mercer Mill Road in Elizabethtown.

Applicants must submit all requested forms and documentation to be considered for acceptance. Applications that do not include all information will be considered incomplete and not processed. Also, all documents must be submitted at the same time.

Required forms and documentation are:

** Picture ID verified by driver’s license, passport or other government issued ID.

** Proof of address where disaster occurred. If address is on driver’s license is not current, please provide a utility bill with disaster address visible.

** Printed Copy of FEMA application(s).

** Printed Copy of all FEMA discussion letters (Note: if documents have been misplaced or destroyed, instructions may be provided to retrieve documents from online. An online account will need to be set up, which requires a user name, email address and password.

Applications must be received by 5 p.m. Wednesday, Jan. 25 at 401 Mercer Mill Road in Elizabethtown. Postmark date is not accepted.
