Bladen County Schools will be using Family ID next year to register student-athletes at East Bladen and West Bladen high schools only. All forms will be completed online by the parents and students in order for the students to participate in athletics.
The link below will allow parents to complete the concussion form, medical release, COVID form, and the pre-physical form. However, students will need to bring in a hard copy of their physical once completed by their healthcare provider and kept on file with the school’s Athletic Director.
These forms must be completed before June 30. Fall sports will begin tryouts on August 2.

EAST BLADEN ONLINE REGISTRATION FOR ATHLETICS
Online registration through FamilyID (www.familyid.com). FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for programs, and helps the school to be more administratively efficient and environmentally responsible. When you register through FamilyID, the system keeps track of your information in your FamilyID profile. You enter your information only once for each family member for multiple uses and multiple programs.

(example: Doctor information, Health Insurance Information, Student ID)

Registering as a new user on FamilyID is simple. Follow these steps:
First, create and activate your FamilyID account.
If you have not already done so, create a FamilyID account here.
After you sign up for an account please check your inbox for an Activation Email from FamilyID. You’ll need to click the link in that email to activate your account.
