Transition Clinic - IMG_8939Southeastern Transition and Primary Care Clinic, an affiliate of Southeastern Health, announces extended hours and welcomes additional resident providers.

Beginning June 22, clinic hours will be extended to 7 p.m. on Wednesday and Thursday evenings with care provided by Nurse Practitioner Jocelyn Mattina. The new hours are Monday, Tuesday and Friday from 8 a.m. to 5 p.m.; Wednesday and Thursday from 7 a.m. to 7 p.m.

Currently providing care at the clinic are Dr. Donald Morando and Mattina as well as Family Medicine Residents Drs. Meredith Beeler, Jeremy Hutson, Courtney Maiden, Noeru Okamura, Carlo Puzzo, Lina Qazi, Sean Rea and Timothy Taylor. They will be joined by the following residents on July 1: Drs. Jonathan Baza, Christopher Carroll, Beau Kalmes, John Nguyen, Robert Omogrosso, Ivan Petrich and William Sykes.

“The Family Medicine Residents are licensed physicians, completing three years of their training in the family medicine clinic before going out into their own practice,” said Dr. Morando, who serves as Family Medicine Residency Program Director for Southeastern Health. “They work closely, side-by-side, with the attending physician to assure high quality of patient care, and in a friendly and compassionate manner.”

While Mattina treats only adults, Dr. Morando and the resident physicians provide primary care for infants, children and adolescents in addition to adults. The clinic was established in June 2015 for primary care and to guide patients as they transition from hospital care to care in the home or other appropriate settings such as rehabilitation or assisted living.

Walk-ins are welcome, however, appointments are encouraged. Southeastern Transition and Primary Care Clinic is located at 2905 North Elm Street in Lumberton. For more information or to schedule an appointment, call 910-738-2662.
